Am I eligible?

You may be eligible if you are:

  • A resident of the United States
  • A pregnant woman diagnosed with migraine who has:
    • Taken Nurtec® ODT (rimegepant) during pregnancy or just prior to pregnancy (within 3 days prior to conception)
    • Taken other migraine medications during pregnancy or just prior to pregnancy*
    • Not taken any migraine medications during pregnancy

*Please note: If you have taken certain migraine medications (e.g., Zavzpret, Reyvow®, Ubrelvy®, Aimovig®, Ajovy®, Emgality®, Qulipta™, or Vyepti®) prior to or during pregnancy you may not be eligible to participate. The registry staff can help determine your eligibility if you have taken these medications.

You are not required to provide an alternate phone contact. If you do provide an alternate contact, you should tell that person of your designation. You can ask the study team to remove your alternate contact at any time. Your alternate contact will not have access to your responses to study questions.

Pharmaceutical Product Development, LLC. (PPD) respects your privacy and understands that it is important to you. The information you provide on this form will be used to contact you for the purposes of assessing your eligibility to participate in the MONITOR Pregnancy Registry. This information will be held by PPD and not shared with the study sponsor (Pfizer, Inc.) or any other third party without your permission. In the event you choose not to enroll, Registry staff will request permission to share your contact information with PPD (a Pharmacovigilance vendor hired by Pfizer, Inc). in order for them to contact you for safety follow up about your pregnancy. If you do not provide permission, and do not enroll in the Registry your contact details will be deleted.
